New Client Scheduling Process

Thank you for your interest in scheduling an initial appointment with Sarah. Please be aware that clients are added to the schedule only when Sarah is certain she can accommodate either weekly or bi-weekly follow-up sessions. This often means that spaces are limited. A waitlist may be available. Sarah is always happy to offer referrals to her colleagues when she is unable to accommodate new clients or she does not have the skills to assist with your concern.

Links to the new client forms are provided below as an information resource for prospective clients. People who are scheduled for an initial appointment will receive a link to create a client portal, which will include electronic versions of these forms to complete and sign.
